Those who would like to register online can do so by using the “Registration and Medical Form” and “Submit Payment” sections below.  You will need to physically sign the submitted form when you take your child to practice. We will ensure that the form  is printed out so that you can sign it then. You will also be required to sign a communicable diseases waiver that day.
